Method of submitting application: Directly at the Consular Affairs Division or Application with necessary information by post/by mail/by fax/ or by Regional office-Matara/Kandy/Trinco/Jaffna/Kurunegala

Time to submit application: 0830hrs–1500hrs
Fees paying to obtain this service: None
Period taken to provide the Service: As soon as the court proceedings on the respective case conclude or the foreign employer settles all legal dues
Necessary Supporting documents: The applicant / complainant / relative should meet the subject officer with the proof of identity as the procedure varies according to circumstances of the cases reported. Please view the relevant for specific information.
Application Form: Only request letter for the complaint is required

- Duly filled Application by the NoK
- Request letter of compensation
- Details of the employer
- Service Agreement
- Company Identity Card
- Insurance Details
- Bank Details
- Medical Reports (If Need)
-For a court case –
- Power of Attorney
- Legal Heir Certificate
- Guardianship Certificate (If there are children below 18 years old)
- Pardon Letter (Murder Case)
- Other relevant legal documents (Birth Certificate/ Marriage Certificate etc.)
If court judgment is given as the deceased person is the accused of the case there is no possibility of obtaining compensation.
